Located at 412 W Cloverdale Rd in Scottsboro, AL 35768, Cloverdale Rehabilitation & Nursing Center is dedicated to providing exceptional care and support to our 141 residents. We are currently seeking a passionate and experienced individual to serve as our Social Services Director.
As the Social Services Director, you will play a pivotal role in enhancing the quality of life for our residents by providing compassionate support and connecting them with essential resources. Your expertise and leadership will be instrumental in addressing their psychosocial needs and ensuring their well-being.
Key Responsibilities of a Social Services Director:
Assessing the psychosocial needs of residents and developing individualized care plans to address their emotional, social, and financial concerns.
Providing counseling and support to residents and their families, offering guidance and assistance in navigating healthcare systems and community resources.
Collaborating with interdisciplinary teams to coordinate resident care and facilitate communication between residents, families, and healthcare providers.
Advocating for residents' rights and preferences, ensuring their voices are heard and respected in decision-making processes.
Maintaining accurate and confidential resident records, documenting assessments, interventions, and progress notes in compliance with regulatory requirements.
Conducting regular meetings and educational sessions for residents and staff on topics related to mental health, aging, and social services.
Monitoring and evaluating the effectiveness of social services programs and initiatives, implementing improvements as needed to enhance resident outcomes.
Ensuring compliance with all state and federal regulations governing social services in long-term care settings.
