A direct hire opportunity is available for a Home Infusion Pharmacist role in Prattville, AL, providing specialized pharmacy services in an alternate care setting. This position focuses on delivering safe and appropriate care to patients receiving home infusion therapy.

Position Overview:
The pharmacist will assess therapy appropriateness, dispense medications and supplies, educate patients and caregivers, and monitor therapy progress. Ensuring compliance with all regulatory and accrediting standards is essential in this role.

Key Responsibilities:

• Maintain ethical and compliant pharmacy practice according to policies, regulations, and professional standards
• Evaluate patients suitability for home care and develop individualized therapy plans
• Dispense medications, supplies, and medical equipment accurately
• Monitor lab results, perform pharmacokinetic dosing, and document therapy outcomes
• Educate patients and caregivers on medication use and infusion techniques
• Ensure USP compliance during sterile compounding and clean room activities
• Supervise and delegate tasks to pharmacy support staff, interns, and students
• Participate in patient care conferences and provide drug information as required
• Engage in on-call duties to respond to patient needs after hours

Qualifications:

• Bachelor of Science in Pharmacy; PharmD or Masters degree preferred
• Active pharmacist license in Alabama with no negative history
• Minimum one year recent home infusion experience
• Strong knowledge of IV admixture services and infusion therapy
• Excellent communication, problem-solving, and organizational skills

Schedule:

• Full-time, Monday to Friday
• Typical hours 8:30 AM to 5:00 PM or 9:00 AM to 5:30 PM with flexibility to stay longer as needed
