Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

An opportunity is available for a skilled pharmacist specializing in home infusion therapy to provide expert pharmaceutical care in an alternate care setting. This direct hire position involves delivering safe, effective, and patient-centered pharmacy services in accordance with professional standards and regulatory requirements.

Key Responsibilities:

• Ensure pliance with all relevant laws, pany policies, and professional pharmacy standards.
• Assess patient suitability for home care and individualized therapy plans.
• Create and manage patient medication profiles, including pharmacokinetic dosing and therapy monitoring.
• Dispense medications and supplies accurately and efficiently, including programming and troubleshooting infusion pumps.
• Document all patient interactions, therapy progress, and munication with healthcare providers.
• Oversee pounding activities ensuring adherence to USP standards and safety protocols.
• Supervise pharmacy staff and coordinate workflows to meet patient needs.
• Participate actively in patient care meetings and on-call rotations as required.
• Provide education to patients and caregivers about medication administration and infusion therapy.

Desired Qualifications:

• Bachelor of Science in Pharmacy, Doctor of Pharmacy, or Masters degree in Pharmacy preferred.
• Current pharmacist license in the state of employment.
• Minimum two years recent experience in an acute-care setting or one year in home infusion therapy.
• Strong knowledge of intravenous admixture and therapy services.
• Excellent verbal and written munication skills.
• Effective problem-solving and team supervision abilities.
• Proficient puter skills, including MS Office and pharmacy workflow systems.
