Key Responsibilities:

1. Develop and implement individualized care plans in collaboration with interdisciplinary teams.
2. Evaluate patient progress and adjust care plans to meet evolving needs.
3. Serve as a liaison between patients, families, and the healthcare team to ensure continuity of care.
4. Assess patients' post-discharge needs and coordinate with external providers and community resources.
5. Facilitate smooth transitions to home, assisted living, or other care settings.
6. Educate patients and families about available resources and services to support recovery and well-being.
7. Maintain accurate and timely patient records, ensuring compliance with federal and state regulations.
8. Prepare reports and participate in audits to monitor care quality and adherence to standards.
9. Provide emotional support and counseling to patients and families.
10. Advocate for patient needs and preferences in care planning and discharge processes.

Qualifications:

1. Current Registered Nurse ( RN ) or Licensed Clinical Social Worker ( LCSW ) certification in the state of AL
2. Minimum of 3 years in case management or discharge planning, preferably in a healthcare or long-term care setting.
3. Strong organizational, problem-solving, and decision-making abilities.
4. Exceptional communication and interpersonal skills.
5. Proficiency in electronic medical records and care coordination systems.
