We are seeking a compassionate and organized Case Manager to work in a 12-bed female residential substance use disorder program. This role focuses on helping clients access services and successfully transition back into the community.
Responsibilities:
- Provide case management services to adult females
- Develop and monitor individualized service/treatment plans
- coordinate care with therapists, medical providers and outside agencies
- Assist clients with:
- housing
- Employment resources
- Maintain accurate and timely documentation
- Support clients in achieving recovery and life goal
- Participate in team meetings and collaborate with clinical staff
Qualifications
- Bachelors/master's degree in social work/addiction counselor/ALC/Certified Recovery Support Specialist (CRSS), CADC, or other related credentials.
- Experience with substance use disorder (preferred)
- Valid driver's license and reliable transportation
- Be able to pass drug screening.
