Job Title: PCP

Location: Mobile, AL

Schedule: Full-time

Base Compensation: $$250K or more depending on experience

Sign-On Bonus: $15K

Annual Incentive Bonus : up to 28% if Base Salary

This role functions within a multidisciplinary care model that includes physicians, advanced practice providers, pharmacists, care coordinators, medical assistants, behavioral health professionals, coding support, referral coordination, and additional clinical staff. The structure is designed to support smaller patient panels, allowing physicians to spend more meaningful time delivering high‐quality, patient‐centered care.

Primary Responsibilities

• Provide patient evaluation, diagnosis, and treatment in alignment with established clinical standards.
• Coordinate patient services including specialty referrals, hospital and skilled nursing transitions, durable medical equipment, and home health needs.
• Participate actively in daily care‐team huddles, offering clinical expertise and supporting team-based decision‐making.
• Maintain a professional, collaborative presence and promote a cooperative clinical environment.
• Complete all medical documentation daily and collaborate with coding support to ensure accurate, comprehensive records.
• Adhere to established clinical protocols and organizational policies.
• Review quality metrics, clinical outcomes, and documentation performance with clinical leadership.
• Provide direct patient care complemented by appropriate administrative time (charting, care coordination, team meetings, etc.).
  

Required Qualifications

• MD or DO degree from an accredited medical school.
• Active and unrestricted medical license, or willingness to obtain licensure in the assigned state.
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
• Active, unrestricted DEA license.
• Medicare and Medicaid provider enrollment (or eligibility to obtain).
• Strong interpersonal and communication skills.
• Demonstrated commitment to a team‐based care model.
• Willingness to adapt to and learn value‐based care delivery.
• Strong patient‐service orientation.
• Proficiency with basic computer applications and electronic medical records (EMR).
  

Preferred Qualifications

• Knowledge of HEDIS/STARS quality metrics.
• Familiarity with CAHPS and HOS measures.
• Experience working in underserved or culturally diverse patient populations.
• Experience using EMR systems effectively.
• Background in Medicare Advantage or global risk‐based care models.
