About the Role:

In this remote position, you'll play a vital role in providing comprehensive telehealth support to patients managing chronic conditions. Your primary focus will be engaging with patients through phone calls and text messaging, offering guidance, monitoring their progress, and fostering a supportive and empathetic environment. You'll be a trusted resource, providing personalized care and empowering patients to take control of their health.

Key Responsibilities:

Proactive Patient Engagement: Initiate outbound calls to new and established patients, establishing rapport and building trusting relationships.
Dynamic Text-Based Communication: Engage in meaningful text conversations with patients, addressing their questions, providing support, and fostering a sense of connection.
Symptom Monitoring and Reporting: Closely monitor patient symptoms and behaviors, accurately documenting observations and promptly reporting any significant changes to the appropriate healthcare professionals.
Collaborative Care Coordination: Communicate effectively with collaborating physicians and specialists, ensuring seamless continuity of care and facilitating timely referrals.
Patient Education and Empowerment: Educate patients about health maintenance, disease prevention, and self-management strategies, empowering them to make informed decisions about their health.
Referral Facilitation: Facilitate referrals to other healthcare professionals and medical facilities as needed, ensuring patients have access to the appropriate resources.
Accurate Documentation: Maintain meticulous and accurate patient medical records, ensuring compliance with all relevant regulations and standards.
Emotional Support and Guidance: Provide compassionate emotional support and guidance to patients and their families, fostering a sense of understanding and empathy.
Creating a supportive environment: Be a friendly and supportive presence to patients.
Qualifications:

Valid and active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.
Proven experience in nursing or other relevant medical fields.
Proficiency in using medical software and equipment, with a strong aptitude for learning new technologies.
Exceptional ability to build rapport and establish trusting relationships with patients from diverse backgrounds.
Strong problem-solving and critical-thinking skills, with the ability to assess situations and make sound decisions.
Ability to thrive in a fast-paced and dynamic remote work environment.
Excellent verbal and written communication skills, with a friendly and professional telephone manner.
A dedicated and quiet home office environment conducive to focused work and confidential patient interactions.
Experience with telehealth is a plus.
